Terms and Conditions

These Terms and Conditions govern your use of services provided by Spark Me IV LLC (“Company,” “we,” “us,” or “our”). By booking, purchasing, or receiving services, you agree to these Terms.
1. Services Provided
Spark Me IV LLC provides mobile IV hydration, vitamin therapy, injections, and wellness services administered by licensed professionals under medical supervision in accordance with Florida laws.
All services are elective wellness services and are not intended to diagnose, treat, cure, or prevent disease.
2. Eligibility
• Must be at least 18 years old
• Complete required intake forms
• Provide accurate medical history
3. Medical Clearance
All services require medical clearance. Good Faith Exams and telehealth evaluations may be performed by Qualiphy.
4. Payment Policy
Payment is due at the time of service. We do not accept insurance.
5. Cancellation Policy
24-hour notice required. Late cancellations and no-shows may incur fees.
6. Refund Policy
All sales are final. No refunds after services are rendered.
7. Limitation of Liability
To the fullest extent permitted under Florida law, Spark Me IV LLC shall not be liable for damages resulting from undisclosed medical conditions, failure to follow instructions, or independent medical decisions.
